Chapel
Located in room 1140 in the Medical Center. Open from 6 am-9 pm. Chaplains are available to both patients and family members. To request pastoral care service call 312.996.0296.

24 hour security provided to visitors & patients of UIMC. Please call 312.966.2830 for additional information.

Billing & Financial
Visit MY UIMC for online bill payment. For any billing questions call 312.996.1000 or visit the Patient Accounts Department at 809 S. Marshfield, 9th floor.

Medical Record Inquiries
In order to receive copies of your medical records, please complete a valid Authorization To Release Health Information Form and return it with the appropriate fees. You can either download the form from the link below or obtain it by contacting the Health Information Management Department at 312.996.6830 (Hours of Operation: 8:00am-4:30pm, Monday-Friday).

Please note: Processing fees, as defined by Illinois State Laws, may apply.
You need to request Radiology films and CDs through the HIM Department as well.
If you are not requesting specific documentation, you will be provided with an abstract which usually includes history and physical, consultations, operative reports, discharge summary, radiology reports, pathology reports, specialized tests, last two days of progress notes and physicians' notes and last two days of lab results.
