To provide us with your NPI, please email us at provider@uic.edu and we will work with you to obtain the required information.
You do not have an NPI or we were unable to apply for one on your behalf. If your NPI is not in the above file, and you are not sure whether or not you have an NPI, please contact us at
provider@uic.edu and we will help you.
Of course, you can always Apply Online for an NPI directly. If you choose this process, we will still need your NPI information. Please forward the letter you receive from NPPES to
provider@uic.edu.
