Established Residents who started before June 1, 2007If you’re an established resident who started before June 1, 2007, please verify whether or not you have a National Provider Identifier. If not, the Graduate Medical Education Office and the Patient Accounts Department are currently working together to establish an NPI for you. We are still in the process and will provide this information as soon as possible. Please come back and check periodically to see if an NPI has been established for you. You can Apply Online for an NPI directly. If you choose this process, we will still need your NPI information. Please forward the letter you receive from NPPES to provider@uic.edu. If you choose this option please be sure to select the correct taxonomy code. As a resident, you must identify yourself as “390200000X – Student in an Organized Health Care/Education/Training Program.” |
